BANU KUMBAK AYGÜN, in our capacity as Data Controller, we hereby inform you of our mutual rights and obligations regarding your personal information, which may be recorded, stored, updated, disclosed to third parties where permitted by legislation, transferred, classified, and processed in the manners specified in the KVKK within the framework explained below.

Purposes and Legal Grounds for Processing Personal Data:

To establish and execute the Doctor-Patient relationship within the scope of legal legislation; to fulfill contractual provisions and provide you with our services; to record identity, address, tax number, and other necessary information, including personal health data, to identify the person performing/requesting the transaction; to arrange all records and documents that will serve as the basis for transactions in electronic (internet/mobile, etc.) or paper environments; to comply with information storage, reporting, and disclosure obligations prescribed by legislation, authorized institutions, and other authorities; and to increase service quality through marketing and statistical activities and offer requested/other products/services. Sensitive personal data are processed due to communication, information provision, and similar processes within the contractual relationship.

Persons/Organizations to Whom Personal Data May Be Transferred for the Above-Mentioned Purposes:

Individuals, public institutions, and organizations, as well as private/public entities permitted by legal legislation provisions. Sensitive personal data may be transferred to domestic and international companies from which we receive services to conduct activities subject to the purposes specified in the legislation we are bound by, secured through confidentiality agreements. Personal and sensitive personal data are stored in a secure environment not open to general use and are strictly not shared with third parties unless authorized or under a legal obligation.

Your Rights Under Article 11 of the KVKK:

By applying to us, you have the right to: a) learn whether your personal data is processed, b) request information if it has been processed, c) learn the purpose of processing and whether it is used in accordance with its purpose, d) know the third parties to whom it is transferred domestically/abroad, e) request correction if it is incomplete/incorrectly processed, f) request the deletion/destruction of data within the framework of the conditions provided in Article 7 of the KVKK, g) request notification to third parties of the transactions made pursuant to items (e) and (f), h) object to the occurrence of a result against you due to analysis exclusively through automated systems, and i) demand compensation for damages in case of loss due to unlawful processing.
